Insurance Information

We are one of the few orthodontists that are approved to accept all insurances including all HMO and PPO plans.  We will be happy to contact and verify your insurance benefits and handle all of the paperwork and explain what your benefits are for orthodontics.  After all the paper work is completed and sent to our central office for processing, we would submit a claim to your insurance company in order for them to cover their portion. 
***Insurance claims are sent to PPO plans only.***

On the day of your appointment, ensure you provide the following information to our office in order to submit a claim to your insurance:
  • Full Name (subscriber and dependent, if applicable)
  • Current address
  • Date of Birth (subscriber and dependent, if applicable
  • Social Security number
  • Employer name
